The therapist/client relationship is based on trust. Practitioners under the Federation of Holistic Therapists (FHT) must protect the client's personal information against improper disclosure or use.
As a professional and a practitioner under the FHT, it is crucial that I uphold the principle of confidentiality when dealing with clients. All information that the client shares with me will be regarded as confidential and used solely for the intended purpose.
In the case of any rare case where my work may involve collaborating with others who may require access to the client's records, it is essential to ensure that proper data protection measures are in place and to inform the client of who will have access to their personal information and why. It is imperative to obtain the client's express permission before sharing any of their details with a third party.
However, in exceptional cases where the client's safety is at significant risk, such as if there are concerns for the welfare of a vulnerable adult or child, or if the client is in real danger of suicide, it may be necessary to share their personal details with the relevant authorities, such as social services or emergency services. It is important to note that any such disclosures should only be made when necessary and in adherence to the appropriate guidelines and regulations.

Resources and support for clients who are depressed or who reveal suicidal intentions:
I hold a Combined Medical Malpractice, Public and Products Liability insurance through the FHT, therefore it is required that I keep client records (which contain personal data) for a minimum of 10 years. If a client is a child, records must be kept for at least 10 years after they reach adulthood.
The client has the right to access all data I hold about them, which is known as a ‘subject access request'. I will supply this information free of charge, in a commonly used format, within one month of the request.
My client records will be legible, professional and in the English language. It is essential to keep full and accurate records for all clients, including details regarding every treatment they have had and any products or equipment used as part of those treatments. These records may be required in court to justify my actions or decisions in the event of a dispute or potential insurance claim.
It is important that client records are reviewed and regularly updated, this will be done at every treatment. When changes or additions are made to any client records, I will date and initial them, and the client will do the same where appropriate. Even if there are no changes to client details or health status between treatments, the client will be required to sign the consultation form at each appointment, to demonstrate that the information has been reviewed and remains current.
